How Much to Tip a Massage Therapist

At a spa, a hotel or a franchise studio, 15 to 20% of the service price is standard. On a $100 hour-long massage that is $15 to $20.

Clinical massage is different, and the distinction is sharper than most guides make it. If your massage was prescribed by a physician, delivered under a treatment plan, or billed through insurance at a physical therapy or chiropractic practice, tipping is generally not expected and is sometimes formally discouraged. The therapist is a healthcare provider in that setting.

One question separates them: was there a prescription, a referral, or an insurance authorisation? If yes, do not tip. If you booked it yourself, chose the session and paid out of pocket, tip 15 to 20%.

Spa or Clinical: How to Tell

The referral question settles almost every case, but a few other signals point the same way.

How the appointment is paid for is the clearest. If it goes through insurance, or the practice issues a receipt with a treatment code on it, you are in a clinical setting. If you paid a spa menu price out of pocket, tip.

Language is another clue. Spas talk about relaxation, aromatherapy and add-ons; clinical practices talk about treatment plans, sessions and referrals. Somewhere with a locker room, robes and a relaxation lounge is a spa.

Franchise studios sit firmly on the spa side despite membership language that can sound medical. Therapists there are paid per session at a modest rate and tips are a real part of their income.

Hotel and Resort Spas: Check for a Service Charge

Resort and hotel spas very frequently add an automatic service charge of 18 to 22% to every treatment. It appears on the bill as a service charge or spa gratuity, and it usually goes to the therapist.

If that charge is present, you have already tipped. Adding another 20% means paying roughly 40%, which is generous but rarely what people intend. Read the bill before signing.

Some resorts also add a separate facility fee for use of the pool, sauna and steam room. That is not a tip and does not reach the therapist, so it does not count toward what you leave.

Couples Massages, Groups and Cancellations

A couples massage involves two therapists and the tip should be split between them. If the room was $240 and you want to leave 20%, that is $48 total, or $24 each — handed to each therapist or clearly marked on the bill.

Group bookings and spa parties often carry an automatic gratuity, the same way large restaurant parties do. It should be disclosed when you book.

Late cancellations are usually charged at the full service price. That fee typically compensates the business rather than the therapist, so there is nothing to tip on — you never received the service.
